Documents and Forms

After you submit the Free Application for Federal Student Aid (FAFSA®) and complete your PA State Grant application items, you may need to submit additional documents and forms if you want to request reconsideration of your PA State Grant eligibility.

Supplemental forms used to determine your 2024-25 PA State Grant eligibility will be available in the PA State Grant System later this summer. Check out our PA State Grant System resources if you need help.

ADA Accommodation Request

PA State Grant Program Distance Education Policy and Procedure for Students with Documented Disabilities (PDF)

If you are:
A PA State Grant recipient at a non-headquartered and domiciled institution and enrolled in an approved program but unable to meet the 50% classroom instruction requirement due to a medical disability

You may either:

  • Send us a message via the PA State Grant System Message Center to request that we send this form to you once it becomes available.
  • Wait for your postsecondary school to report this information to us. Once your school reports this information, we will automatically assign you a form for completion.
